Accident Reporting & Issues

Accident Reporting & Issues gives staff, students, contractors and visitors one place to report accidents, incidents, near misses, hazards and other concerns. They can report in SOTERweb or, without a login, from an intranet link or a QR code on site. Each report goes to an Issue Manager, usually the department's health and safety representative, to review, investigate where needed and close.

You choose the issue types, the sections each one asks for and who is emailed when each is reported. Issues with several people injured escalate automatically, and other escalation rules can be added during set-up. Investigations record the team, contributing factors, control measures and action taken, and follow-up actions are tracked to closure with email reminders. Dashboard tiles and Power BI reports show status and trends, and the module also includes a Security Officers Log and a Fire Officer Log.
